- A prescription is required only if submitting an insurance claim for coverage under your extended health plan.
- Download our Prescription Form and have it filled out/signed by a physician.
- The list of insurance providers is not exhaustive. To confirm whether your garments will be covered under your extended health plan, inquire with your insurance provider.
- Yes, we accept payments from customers in the form of cash, credit and electronic transfers.
